Programs & Services: CYFAN

 

Children, Youth & Family AIDS Network

Connecticut Primary Care Association (CPCA) currently receives funding under the Ryan White Care Act Part D to administer the Children, Youth and Family AIDS Network (CYFAN) of Connecticut. This statewide provider network is supported on a contractual basis by CPCA for medical care, family-centered case management services and outreach. CPCA fulfills its role in the administration, coordination, program development and reporting functions of CYFAN through day-to-day management of subcontracts with 6 federally qualified health centers and 3 hospitals. CYFAN maintains a presence on the agency website www.ctpca.org (this site) and a special adolescent focused website dedicated to CYFAN www.ctyouthhiv.com.

The six community health centers that participate in CYFAN all administer comprehensive culturally competent HIV/AIDS outreach services that are driven by case finding and appropriate linkages to care for youth between the ages of 13-24. This youth outreach component , originally funded in 1999 under the Minority AIDS Initiative of Ryan White Care Act , requires each health center to continually demonstrate its unique ability to positively impact the health and well being of historically underserved HIV/AIDS populations in various target communities.
