Southwest Community Health Center, Inc.

Called the "Miracle on Bird Street" when it was established
in 1976, Southwest Community Health Center has grown to
multiple sites providing comprehensive health care services. The
primary site at 361 Bird Street is located adjacent to the P.T.
Barnum Public Housing Complex; Marina Village is convenient to
the city’s largest public housing complex; and Clinton Avenue
is located in a neighborhood of Victorian homes, many of which
are now professional offices.
Affiliated with Bridgeport Hospital, Yale-New Haven Hospital
and St. Vincent’s Medical Center, Southwest Community Health Center
is a nurse practitioner and nursing school clinical training site
for area universities. With a staff of physicians, dentists, nurse
practitioners, dental hygienists, social workers and certified
drug/alcohol counselors, Southwest provides comprehensive adult
medicine, ob-gyn, pediatrics, adolescent medicine, dentistry and
mental health services.
Southwest Community Health Center serves over 12,000 medical
patients, more than 3,000 dental patients, and provides more than
8,000 behavioral health visits annually. The population served
by the Center, based on the total Bridgeport population, is approximately
43% Black or African American, 42% Hispanic or Latino, 15% White,
Special programs at Southwest Community Health Center include
comprehensive care to pediatric and adult patients with HIV/AIDS;
the Michele Project, a pioneer program for early detection of
major cancers; Project Connect to screen for victims of domestic
violence; a prescription medications voucher program in conjunction
with Pfizer Pharmaceuticals; and comprehensive outpatient services
for substance abuse issues.
