Children, Youth & Family AIDS Network


The Children, Youth & Family AIDS Network provides family support/case management services to HIV infected/exposed and affected children and their families in the cities of Stamford, Bridgeport, New Haven, and Hartford. In addition, program staff provides family support/case management services to HIV infected/exposed children and their families who live outside of Hartford but receive their pediatric HIV medical care at the Connecticut Children's Medical Center. At its affiliated sites, the Children, Youth & Family AIDS Network promotes coordination of primary and tertiary care, access to pediatric clinical trials, and provides or facilitates access to needed concrete or psychosocial services. The Program provides coordination of maternal-child health care and facilitates early entry into care for HIV positive women. Program teams in the four cities consist of community based family support workers/case managers, clinical coordinators, and pediatric HIV medical providers.
