Hill Health Center was established in 1968 in a collaboration between the community and Yale Medical School. The first community health center in Connecticut, Hill Health Center serves New Haven neighborhoods which are among the poorest in the state. Hill Health Center also provides health care services to the city of West Haven and towns of the Lower Naugatuck Valley – Ansonia, Derby, Seymour, Shelton, Naugatuck and Oxford.

Hill Health Center is affiliated with Yale-New Haven Hospital and the Hospital of St. Raphael. Staff includes internists, OB-GYNs, pediatricians, psychiatrists, nurse practitioners, physician assistants, nurse midwives, dentists and dental hygienists provide care to approximately 18,300 patients. Executive Director Cornell Scott serves as an adjunct professor of public health at Yale School of Medicine.

Hill Health Center, through its satellite sites, serves a geographically diverse patient population. The patient population is also ethnically diverse: 45% African-American, 31% Latino, and 24% White, other.

Winner of the 1997 Johnson & Johnson Crystal Award, Hill Health Center provides comprehensive health care. Special programs include: outreach to homeless individuals and families; birth-to-three services for developmentally delayed children; school-based health centers; outreach and social/educational services to pregnant teenagers; emergency food pantry; AIDS education and outreach to at-risk populations; 24-bed medical detoxification program; 31-bed shelter for homeless men; and a child guidance clinic.

Eye Clinic Now Open

The Eye Clinic was reopened thanks to the cooperation of the Yale Opthalmology Department and a gift from the Community Foundation. The Eye Clinic is located where the Billing Department formerly was. Billing relocated to the Dixwell Avenue site. Having the Eye Clinic here makes it much more convenient for patients who are referred for ophthalmology services.
